Is this testing or therapy?
SK Wellness Services is primarily focused on psychological testing and evaluation. We also offer employer services and other specialized services.
Who do you evaluate?
We evaluate adolescents and adults, as well as selected pediatric referrals when appropriate.
What kinds of evaluations do you provide?
Comprehensive psychological evaluations, ADHD evaluations, diagnostic clarification, learning/cognitive assessment, autism evaluations when appropriate, immigration evaluations, bariatric evaluations, psychosocial assessments, fitness-for-duty, and selected forensic/legal evaluations. Do you accept insurance?
We accept self-pay and most major insurance panels. An individual self-pay rate and a sliding scale are also offered. Please contact the office to verify your benefits and discuss fees.
I recently lost my health insurance. Can I still afford an evaluation?
Yes. We offer a sliding scale for clients paying out of pocket, based on your budget. Fees depend on the type of evaluation and the extent of testing, so contact the office and we'll work out a fee that fits. See our Sliding Scale page for details.
Are services in person or virtual?
Both. Telehealth appointments are available throughout Texas when clinically appropriate. Initial interviews, feedback sessions, and certain evaluation services may be completed by secure telehealth, while some testing may require an in-person visit depending on the referral question and measures used.
What tools do you use?
The battery is individualized to the referral question. Depending on the concern, testing may assess cognitive functioning, academic functioning, attention, executive functioning, personality, behavioral and emotional functioning, adaptive functioning, and other symptom-specific areas.
How experienced is the practice?
The practice is led by Shannon Khan, M.A., LPA, LPC, who has 20+ years of clinical and psychological assessment experience across medical, school, forensic, community, and private-practice settings.
What areas do you serve?
Our office is in the Houston / Clear Lake area, serving Webster, Clear Lake, League City, Friendswood, Nassau Bay, Seabrook, Kemah, Pearland, Pasadena, and the greater Houston area. Telehealth is available throughout Texas when clinically appropriate.
How long does an evaluation take, and how much does it cost?
The timeline and cost depend on the referral question and the testing required, and insurance coverage varies by plan. Please contact the office to discuss the specifics of your evaluation.
How do I get started?
First, contact SK Wellness. The referral is reviewed for appropriateness, intake forms are sent, insurance benefits are verified when applicable, and an initial appointment is then scheduled.
